Abstrak

Introduction: Vulvar cancer is a relatively rare malignancy, representing only 4% of all gynecologic cancers. Often asymptomatic in its early stages, vulvar cancer typically requires surgery for management, particularly for early-stage cases. Treatment often involves a radical vulvectomy followed by inguinal lymph node dissection through a separate incision. This case aims to provide insights into the early detection and comprehensive management of vulvar cancer. Case Presentation: A 67-year-old woman was referred from Gambiran General Hospital Kediri to the Saiful Anwar General Hospital oncology clinic following a vaginal biopsy that confirmed vulvar cancer. The patient presented with pain in the pubic area and tissue growth around the pubic region since August 2023. She reported a weight loss of 10 kg over the past year and has been menopausal for the past 18 years. Vital signs were as follows: BP 132/79 mmHg, pulse 128/min, RR 20/min, temperature 36.5°C, height 155 cm, weight 40 kg, and BMI 16.64 kg/m² (underweight). Physical examination revealed a 12x8 cm mass on the right vulva with an odor. The vulvar biopsy on 25/08/2023 confirmed well-differentiated squamous cell carcinoma. The patient is planned for elective-wide excision. Management included radical vulvectomy with V-Y flap reconstruction surgery. Conclusion: Vulvar cancer, though relatively rare, often presents as squamous cell carcinoma (90-95% of cases). This patient’s treatment plan involved radical vulvectomy with V-Y flap reconstruction. Early detection and comprehensive management are crucial for improving outcomes in vulvar cancer.
